Amends the Medical Assistance Article of the Illinois Public Aid Code. In provisions concerning the Medicaid Access Adjustment payments to nursing facilities, provides that for dates of service beginning January 1, 2027, the Medicaid Access Adjustment shall be increased by $5.75 to $10.50 per diem. Provides that beginning January 1, 2027, facilities located outside of Rate Areas 6, 7, and 8 shall have the Medicaid percent of occupied bed days to be at least 60% of all occupied bed days adjusted quarterly to qualify for the Medicaid Access Adjustment. Provides that the remaining facilities shall have their threshold remain at 70%.

Congratulates Richard "Rick" Kwasneski on his retirement from public service during the summer of 2026. Thanks him for his profound contributions to the infrastructure and economy of northeastern Illinois. Wishes him the very best in his future endeavors.

Declares October 11, 2025 as Beautiful Minds Pink Saree Day/#MyPinkHealth Awareness Day in the State of Illinois. Encourages all residents of Illinois to support initiatives that advance women's wellness, mental health, and youth empowerment.

Appropriates $500,000 from the General Revenue Fund to the Department of Early Childhood for grants and administration expenses associated with support for the inclusion of children ages 3 to 5 with developmental delays and disabilities in school and community-based early childhood settings. Effective July 1, 2026.

Amends the Smoke Detector Act. Bans, beginning December 31, 2027, the selling, offering for sale, or giving as a gift a smoke detector that is not designed to receive primary power from the building wiring or does not meet certain battery requirements set forth in the Act. Specifies that this does not prohibit manufacturing within the State and distributing, selling, offering for sale, or giving as a gift outside the State any smoke detector. Specifies that a person who violates this requirement is guilty of a petty offense and is subject to a fine. Defines "person" within certain provisions of the Act.

Creates the Firefighter Paid Family Leave Act. Provides that a firefighter shall receive 6 weeks of paid family leave that may be used: (1) for the birth of a child in order to care for the child; (2) to care for a newly adopted child under 18 years of age, a newly placed foster child under 18 years of age, or a newly adopted or placed foster child older than 18 years of age if the child is incapable of self-care because of a mental or physical disability; and (3) to care for a family member with a serious health condition. Provides that an employer shall compensate a firefighter granted leave under the Act at the firefighter's regular rate of pay, paid at the same interval the firefighter was paid while working, for those regular work hours during which the firefighter is absent from work. Sets forth provisions concerning employment protection and maintenance of benefits; prohibited acts, limitations of the Act; waiver of leave under the Act; and collective bargaining agreements. Limits home rule. Provides that the Department of Labor may adopt any rules necessary to implement the Act. Effective immediately.

Amends the Grant Accountability and Transparency Act. Provides that the advice and technical assistance provided to State grant-making agencies by the Governor's Office of Management and Budget for the purpose of ensuring compliance with the Act shall include training for State agency staff. Sets forth requirements for the training. Provides that a State grant-making agency that awards a grant must issue the grant agreement associated with the grant within 60 days after the grant's effective date. Sets forth requirements for the grant agreement.

Amends the Accident and Health Article of the Illinois Insurance Code. Provides that an individual or group policy of accident and health insurance that is amended, delivered, issued, or renewed on or after January 1, 2028 shall cover a medically necessary coronary calcium scan and scoring every 36 months for individuals over the age of 40. Defines "coronary calcium scan and scoring". Makes conforming changes in the State Employees Group Insurance Act of 1971, the Counties Code, the Illinois Municipal Code, the School Code, the Health Maintenance Organization Act, the Limited Health Service Organization Act, the Voluntary Health Services Plans Act, and the Medical Assistance Article of the Illinois Public Aid Code. Effective January 1, 2027.

Creates the Healing Through History Act. Requires the Department of Healthcare and Family Services to provide prospective adoptive families, no less than 30 days prior to adoption finalization, with a complete, unredacted copy of the child's full case record, including, but not limited to: (i) all placement histories and foster home records; (ii) all caseworker notes and observations; (iii) all educational, psychological, and behavioral reports; (iv) all therapy or counseling notes, subject to certain federal requirements; and (v) all medical records in full, including historical records. Provides that former foster youth aged 18 or older are entitled to receive the same information and records at no cost in both physical and digital form. Contains provisions concerning ongoing record access post-adoption or post-emancipation; the development of Department protocols and infrastructure necessary to collect, preserve, and transmit full child records; exemption and confidentiality protections; federal disclosure requirements; and other matters. Effective January 1, 2026.

Appropriates $1,000,000 from the General Revenue Fund to the Department of Human Services for a grant to Special Olympics Illinois for costs associated with contingent and operational expenses. Effective July 1, 2026.
