Rep. Keith P. Sommer

Amends the Illinois Insurance Code, Health Maintenance Organization Act, Limited Health Service Organization Act, and Voluntary Health Services Plans Act. Provides that no insurer may issue a service provider contract that requires an optometrist or ophthalmologist to provide services or materials to the insurer's policyholders at a fee set by the insurer unless the services or materials are covered services or materials under the applicable policyholder agreement. Effective January 1, 2014.

Amends the Animal Control Act. Provides that a County Animal Control that is located in a county that has been declared a disaster area under the Disaster Relief Act due to a natural disaster, or in a county which a part of the county has been declared a disaster area under the Disaster Relief Act due to a natural disaster, shall retain impounded animals for twice its normal retention time or for a minimum of 14 days after the disaster declaration, whichever is longer, before the County Animal Control may proceed with the adoption, transfer, or euthanasia of the impounded animal. Provides that a County Animal Control shall keep a detailed list of the impounded animals that were transferred to another facility for a minimum of one month after the disaster. Provides that facilities receiving transferred animals shall retain the impounded animals for twice its normal retention time or for a minimum of 14 days after the disaster declaration, whichever is longer. Effective immediately.

Amends the Illinois Municipal Code. Makes a technical change in a Section regarding cemeteries.

Amends the Animal Control Act. Provides that a County Animal Control that is located in a county that has been declared a disaster area under the Disaster Relief Act due to a natural disaster, or in a county which a part of the county has been declared a disaster area under the Disaster Relief Act due to a natural disaster, shall retain impounded animals for a minimum of 3 days beyond its normal retention time before the County Animal Control may proceed with the adoption, transfer, or euthanasia of the impounded animal. Provides that a County Animal Control shall keep a detailed list of the impounded animals that were transferred to another facility for a minimum of one month after the disaster. Provides that facilities receiving transferred animals shall retain the impounded animals for an additional 3 days beyond its normal retention time. Effective immediately.

Amends the Cemetery Oversight Act. Makes a technical change in a Section concerning the short title.

Amends the Counties Code. Provides that a county board shall name (currently, may name) any street, lane, road or highway, and shall regulate (currently, may regulate) the numbering of buildings and lots adjacent to any street, lane, road or highway. Provides that no change in name or numbering shall be effective until 30 days after the following entities having jurisdiction over the area are notified: (1) election authorities; (2) post office branch; and (3) 9-1-1 system. Requires the change in name or numbering to be effective no less than 30 days prior to any election. Makes similar changes in the Illinois Municipal Code.

Amends the Freedom of Information Act. Provides that, when contained in an employment record of a law enforcement agency, including, but not limited to an application for employment, the name, address, and contact information of a law enforcement officer and his or her immediate family members shall be deemed personal information the disclosure of which would constitute a clearly unwarranted invasion of personal privacy, and that information shall be exempt from disclosure under the Act. Defines "immediate family members", "law enforcement agency", and "law enforcement officer". Effective immediately.

Amends the State Designations Act. Designates pumpkin pie as the official State pie of the State of Illinois.

Amends the Workers' Compensation Act. Provides that, in computing the compensation to be paid by an employer having 50 or fewer employees, "average weekly wage" does not include benefit pay for holidays or for sick, personal, or vacation days.

Congratulates Paul and Barbara Segobiano on the occasion of their 58 years of dedicated service to the Bloomington Knockers Youth Football Team.
