Amends the Illinois Act on the Aging. Provides that, subject to federal approval, on and after January 1, 2027, rates for in-home services shall be increased to $33.92 to sustain a minimum wage of $20.75 per hour for direct service workers. Requires rates in subsequent State fiscal years to be no lower than the rates put into effect upon federal approval. Provides that in order for a provider of in-home services to be eligible to receive the $33.92 rate, the provider must pay a minimum wage of $20.75 per hour to all direct service workers employed by the provider. Requires providers of in-home services to certify to the Department on Aging that they remain in compliance with the mandated wage increase for direct service workers. Requires each provider of in-home services to submit cost reports to the Department consistent with a specified administrative rule in order to be eligible for the $33.92 rate for in-home services. Provides that fringe benefits shall not be reduced in relation to the described rate increases. Requires the Department, beginning January 1, 2027, to ensure that each in-home service provider spends a minimum of 79% of the total payments the provider receives for home care aide services it furnishes under the Community Care Program on total compensation for direct service workers who furnish those services. Requires annual direct service worker cost reports from providers; and requires the Department to amend existing rules on financial reporting and minimum direct service worker costs to reflect the increase in the direct service worker spending requirement from 77% to 79%.

Amends the Community-Integrated Living Arrangements Licensure and Certification Act. In provisions concerning per diem reimbursement for a community-integrated living arrangement provider, provides that, if it is determined by third-party medical personnel that a resident that required a medical absence is unable to return to the community-integrated living arrangement, or if the resident dies during the medical absence, the provider shall receive 100% of the per diem reimbursement for up to 20 medical absence days. Effective immediately.

Amends the Medical Assistance Article of the Illinois Public Aid Code. Provides that, subject to federal approval, beginning January 1, 2027, any individual age 22 to 64 who is diagnosed as having Alzheimer's disease or a related dementia and is determined to be a person with a disability by the Social Security Administration shall be eligible for services in a supportive living dementia care setting if the individual meets all other eligibility requirements to receive services in a supportive living dementia care setting in accordance with specified administrative rules. Requires the Department of Healthcare and Family Services to apply for any federal waiver necessary to implement the amendatory Act. Effective immediately.

Amends the Illinois Library System Act. Increases the amounts to be provided by the State Librarian for annual library system grants, for annual grants to qualified public libraries in the State, and for annual school library grants. Effective immediately.

Amends the Department of Natural Resources (Conservation) Law of the Civil Administrative Code of Illinois. Provides that, subject to appropriation, the Department of Natural Resources shall design, implement, and evaluate not more than 3 demonstration projects that test natural flood risk reduction practices in flood-prone areas of the State, as determined by the Department, with certain requirements. Provides that the Department may require a county to submit to the Department a progress report with certain information. Provides that the Department shall submit to the General Assembly and to the Illinois Emergency Management Agency and Office of Homeland Security a report with certain information. Allows the Department to adopt rules.

Amends the Program of All-Inclusive Care for the Elderly Act. Provides that the Department of Healthcare and Family Services shall coordinate with the Department on Aging and the Department of Human Services to ensure the maximization of all available federal financial participation and existing State revenue sources, which shall include, but not be limited to, identifying and integrating funding streams currently used for the Home and Community-Based Services (HCBS) waivers to support PACE enrollment and developing a unified budgeting approach under which appropriations for long-term services and supports are treated as a fungible pool, allowing funding to transition seamlessly when a participant chooses PACE over traditional waiver services. Provides that, to ensure participant choice and program flexibility, the Department shall establish a service-neutral enrollment mechanism. Provides that if an individual is on a waiting list for a HCBS waiver and chooses to enroll in PACE, the individual's status and slot value shall be preserved and applied to the PACE capitation rate to ensure the State's budget neutrality. Sets forth provisions concerning funding portability, transitioning between PACE and traditional HCBS models, and the use of a Unified Assessment Tool.

Creates the Natural Organic Reduction Regulation Act. Provides that any person doing business in this State, or any cemetery, crematory, funeral establishment, corporation, partnership, joint venture, voluntary organization, or any other entity, may erect, maintain, and operate a natural organic reduction facility in the State and provide the necessary appliances and facilities for the natural organic reduction of human remains in accordance with the Act. Provides that an individual or a person, cemetery, crematory, funeral establishment, corporation, partnership, joint venture, voluntary organization, or other entity may reduce human remains only in a natural organic reduction facility operated by a disposition authority licensed for this purpose and only under the limitations provided in the Act. Provides for: grounds for denial or discipline; surrender of a license; license, display, transfer; authorizing agent; authorization for natural organic reduction; performance of natural organic reduction services, training; recordkeeping; natural organic reduction procedures; disposition of reduced human remains; limitation of liability; hazardous implants; penalties; failure to file an annual report; injunctive action, cease and desist order; service of notice; investigations, notice, and hearing; compelling testimony; administrative review, venue, certification of record, and costs; and preneed of natural organic reduction arrangements. Effective June 1, 2026.

Amends the Mental Health and Developmental Disabilities Administrative Act. Requires the Department of Human Services to establish reimbursement rates that build toward livable wages for frontline personnel in residential and day programs and service coordination agencies serving persons with intellectual or developmental disabilities. Provides that for community-based providers serving persons with intellectual or developmental disabilities, subject to federal approval of any relevant Waiver Amendment, the rates taking effect for services delivered on or after January 1, 2027 shall include an increase in the rate methodology sufficient to provide a $1.20 per hour wage rate increase for all direct support professionals and all other frontline personnel who are not subject to the Bureau of Labor Statistics' average wage increases and who work in residential and community day services settings. Prohibits community-based providers from using the rate increases for operational or administrative expenses. Requires rate increases that are sufficient to provide wages for all residential non-executive direct care staff. Amends the Medical Assistance Article of the Illinois Public Aid Code. Provides that the rates for ID/DD and MC/DD facilities for services delivered on or after January 1, 2027 shall include a $1.20 per hour wage increase for all direct support professionals and other frontline personnel who are not subject to the Bureau of Labor Statistics' average wage increases. Provides that changes to the rate methodologies are subject to federal approval. Grants emergency rulemaking authority to the Departments of Human Services and Healthcare and Family Services to implement the rate increases. Effective immediately.

Creates the Operating Room Patient Safety Act. Provides that each surgical technologist hired or contracted by a health care facility on or after January 1, 2027 shall meet specified educational, certification, or experiential requirements. Provides that nothing in the Act prohibits an individual from performing surgical technology services if the individual is acting within the scope of the individual's license or registration or is a student or intern under the direct supervision of a licensed health care provider.

Creates the Firefighter Paid Family Leave Act. Provides that a firefighter shall receive 6 weeks of paid family leave that may be used: (1) for the birth of a child in order to care for the child; (2) to care for a newly adopted child under 18 years of age, a newly placed foster child under 18 years of age, or a newly adopted or placed foster child older than 18 years of age if the child is incapable of self-care because of a mental or physical disability; and (3) to care for a family member with a serious health condition. Provides that an employer shall compensate a firefighter granted leave under the Act at the firefighter's regular rate of pay, paid at the same interval the firefighter was paid while working, for those regular work hours during which the firefighter is absent from work. Sets forth provisions concerning employment protection and maintenance of benefits; prohibited acts, limitations of the Act; waiver of leave under the Act; and collective bargaining agreements. Limits home rule. Provides that the Department of Labor may adopt any rules necessary to implement the Act. Effective immediately.
