Amends the Illinois Public Aid Code. Provides that, subject to the approval of a federal waiver, persons who are 21 years of age or older who have received Medicaid benefits under provisions concerning persons under age 21 who would qualify as disabled shall remain eligible for continued Medicaid benefits, outside an institution, at a level of care appropriate to meet the individual needs of the person, provided that a physician annually determines that the person requires the level of care provided by a hospital, skilled nursing facility, or intermediate care facility. Requires an annual report by the Department of Human Services beginning January 1, 2007. Requires the Illinois Department of Human Services, the Illinois Department of Public Aid, and the Division of Specialized Care for Children of the University of Illinois to enter into an interagency agreement for the purpose of cooperatively establishing a program of case management for any person who receives Medicaid benefits under the provisions concerning persons under age 21 who would qualify as disabled. Sets forth the minimum requirements for case management services. Effective immediately.
Amends the Department of Public Health Powers and Duties Law of the Civil Administrative Code of Illinois. Creates a standing Task Force on Huntington's Disease within the Department of Public Health. Sets forth provisions for the composition of the Task Force and its operations. Provides that Members of the Task Force shall be reimbursed for their necessary expenses incurred in performing their duties and that the Department shall provide staff and technical assistance to the Task Force to the extent possible within annual appropriations for its ordinary and contingent expenses. Sets forth that Task Force duties include, (i) obtaining data and analyses regarding the prevalence and burden of Huntington's disease, (ii) coordinating the efforts of the Task Force with existing State committees and programs providing Huntington's disease education, counseling, and case management, (iii) raising public awareness on the nature of Huntington's disease, the value of early detection, options for testing, treatment costs, new technology, medical care reimbursement, and physician education, (iv) examining the limitations of existing laws, regulations, programs, and services with regard to coverage and awareness issues for Huntington's disease, and (v) receiving and considering reports and testimony from individuals and public and private health organizations to learn more about their contributions to Huntington's disease diagnosis and treatment and more about their ideas for improving Huntington's disease diagnosis and treatment in Illinois. Provides for annual reports to the Governor and General Assembly. Sets forth that the Task Force shall expire on April 1, 2009, or upon submission of the Task Force's final report to the Governor and the General Assembly, whichever occurs earlier. Effective October 1, 2006.

Creates the Community First Act. Provides that all persons who reside in, or are eligible to reside in, institutional care facilities have the right to have the amount of public funds that are or would have been expended on him or her for services provided by an institutional facility transferred to pay for community services. Requires the Department of Human Services, Department of Public Health, Department on Aging, and institutional facilities to inform eligible persons of their right to funding for community services under this Act. Requires the Department of Public Aid and the Department of Public Health to publish, annually, a report describing the implementation of the Act. Provides that the Act shall not alter or affect the manner in which persons with disabilities are determined eligible or appropriate for community services, except to the extent the determinations are based on the availability of community services, and shall not be read to limit in any way the rights of people with disabilities under federal or State law. Requires the Department of Public Aid, in consultation with the Department of Human Services, the Department of Public Health, and the Department on Aging, to adopt any rules necessary for the implementation and administration of this Act.
