Requires every hospital that provides rehabilitation servicesto make arrangements for all necessary follow-up care for a patientprior to discharging such patient, including scheduling initial appointmentsfor any follow-up care and providing the patient with the namesand contact information for each provider and information regardingeach scheduled appointments.
