Establishes the blue-ribbon commission on the future of New York state's service delivery system for individuals with intellectual and developmental disabilities act

Added a requirement that at least one-third of the commission members must be consumer representatives, including individuals with developmental disabilities, parents, or guardians.

Added a requirement that at least one member must be an immediate relative or guardian of an individual with a developmental disability who resides or previously resided in a community-based residential setting.

Reordered and renumbered the list of commission members to accommodate the new requirements, shifting the original list items (d through l) to (c through l) and adding new items (a and b) for consumer representation.

Updated the bill's title and committee assignment status to reflect that it has been amended and recommitted to the Committee on Disabilities.
