Health: immunizations; exemptions to immunization requirements; modify to require the use of forms created by the department. Amends sec. 9215 of 1978 of 368 (MCL 333.9215).

Under the new rules, physicians must use a specific state-created form to certify that a vaccine is medically contraindicated for a child, while parents seeking exemptions based on religious beliefs or other objections must obtain a separate waiver form in person from a local health department. The nonmedical waiver process includes a mandatory step where a local health officer informs the parent of the risks associated with not vaccinating before witnessing their signature. Additionally, both forms will include specific language informing users that children with exemptions may be excluded from school or group programs during disease outbreaks.
