An Act relative to caregiver authorization affidavits

This bill allows parents, legal guardians, or custodians to sign a sworn affidavit designating a caregiver to make specific decisions for their minor children. The designated caregiver gains the authority to consent to medical treatment, access health records, make educational choices, and approve recreational activities for the child. To be valid, the affidavit must include personal details of all parties, confirm no court orders block this arrangement, and be signed before two witnesses and a notary. The authorization is effective for up to two years unless a different timeframe is specified and can be renewed or revoked by the parent.
